- Add PROACTIVELY (uppercase) to test-writer-fixer description
- Ensures consistency with other 6 PROACTIVELY agents
- All 7 coordinators now use uppercase PROACTIVELY keyword
- Add MASTER-PROMPT.md.backup to .gitignore
